Ottawa Korean Community Church (OKCC) Consent Form Regarding COVID-19 Liability Release Waiver

Ontario Government has announced that effective Friday, June 12, 2020, places of worship are permitted to open with physical distancing in place and attendance limited to no more than 30 percent of the building’s regular capacity with some other specific guidelines for the places of worships to adhere to. In compliance with such guidelines, the OKCC has safety measure in place for the physical attendees to the services to be held in the OKCC building. In that regard, the OKCC hereby requires all those who physically attend the Sunday Services and Wednesday Services to read the following and sign if in agreement before entering the premises of the OKCC.

• In consideration of my participation in the foregoing activity in the premises of OKCC, the undersigned acknowledges and agrees to all of the following:

    I have not experienced symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.

    I have not, nor any member(s) of my household, traveled internationally within the past 30 days.

    I have not been, nor any member(s) of my household, diagnosed to be infected of COVID-19 virus within the last 30 days.

    I am aware of the existence of the risk on my physical appearance to the premises of OKCC and my participation to the activity in the premises of OKCC that may cause injury or illness of COVID-19 that may lead to paralysis or death.

    I am fully and personally responsible for my own safety and actions during my participation in the premises of OKCC and I recognize that I may be in any case at risk of contracting COVID-19.

SIGNATURES

 By signing below I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.

 This waiver will remain effective if the above body conditions are kept and until laws and mandates relevant to COVID-19 are lifted.

온타리오 주 정부는 2020 년 6 월 12 일 금요일부터 물리적 거리두기와 예배 참석인원을 예배당 수용인원의 30%이하로 제한하는 것을 비롯하여, 교회가 지켜야 할 정부 지침을 준수하는 경우에만 현장 예배를 위해 교회건물을 개방할 수 있다고 발표했습니다. 이러한 정부의 지침에 따라, 오타와 한인교회는 현장 예배에 참석하는 교인들을 위한 안전 장치를 마련하였으며, 주일 현장 예배와 수요 현장 예배에 참석하시는 모든 성도들은 아래 동의서의 내용을 읽고 동의하시는 경우 서명을 하신 이후에만 예배당에 입장하실 수 있습니다.

상기한 오타와 한인교회 예배당에서의 현장예배 참여에 대해, 본인은 다음 모든 사항을 인정하고 동의 합니다.

o 열, 피로, 호흡 곤란 또는 마른 기침의 증상을 경험하지 않았거나 지난 14 일 이내에 COVID-19 또는 전염병과 관련된 어떤 증상도 보이지 않았습니다.

o 본인 또는 가족중 지난 30 일 이내에 국제 여행을 한 사람은 없습니다.

o 본인 또는 가족중 지난 30 일 이내에 COVID-19 바이러스에 감염 진단을 받은 사람이 없습니다.

o 본인은 오타와 한인교회 건물내에서 열리는 예배에 참석함으로 신체적 위험에 노출 될 수 있음을 인지하고 있으며, 현장예배에 참석함으로 인해 COVID-19에 감염될 경우, 마비와 같은 신체적 손상을 입거나 심한경우, 사망에 이를 수도 있다는 것을 인지하고 있습니다.

o 본인은 오타와 한인교회 건물 내의 활동에 참여하는 동안, 본인의 안전과 행동에 대해 전적으로 개인적으로 책임을 질 것 이며, COVID-19감염의 위험이 있음을 인지하고 있습니다.

서명:

 아래에 서명함으로써 본인은 상기의 법적 책임 면책 동의서를 읽었으며, 그 내용을 이해하고 인정합니다. 본인은 18 세 이상이고, 자의에 의해 이 동의서에 서명할 자격이 있음을 밝힙니다. 본인은 관련된 위험에 대해 충분한 정보를 받았으며, 본인의 자유 의지에 의해 그 누구의 강요없이 자발적으로 위 사항들에 대하여 동의를 표합니다.

 이 권리 포기서는 상기에 명시된 개인의 건강상태에 변화가 없고, COVID-19 관련 법안과 정부의 지침이 해제되기 전까지 유효합니다.
